A Corrective actions can be triggered inside any checklist when something is off. This is the most common way team members create them.
Typical Places They Get Triggered
Examples:
Daily Cleaning Checklist — surface not cleaned properly.
Equipment Startup Log — pressure too low, machine vibrating, alarms showing.
Incoming Goods Log — broken packaging, wrong temperature, foreign objects.
Temperature Log — fridge or freezer out of range.
HACCP Monitoring Checklist — CCP limit exceeded.
Packaging Check — incorrect labels or missing allergen info.
Any “No” answer or out-of-range value will trigger the warning message that will be displayed and will guide you filling a Corrective Action form before continuing with the checklist.

Show every corrective action created in a time period.
Show the related checklist/log.
Show who fixed it.
Show proof of the fix (photos, notes).
Show timestamps and responsibilities.

How to use Corrective Actions:
In Checklist activity click on the "Create Corrective Action" button:
In the opened "Corrective Actions" modal window, type text in the "Title of corrective action" field.
Then type text in the "Description" field to describe the problem more specifically.
Then type text in the "Immediate action/correction" field to tell what needs to be done first af all.
Next type text in the "Root cause" field to tell what is most likely or definitely causing the problem.
Type text in the "Preventive/corrective actions" field to tell what to do to prevent that problem in future.
Also you can attach some files to display the problem.
You can select Lot number if such aplicable.
Then select "Start date" and "End date" for when the problem should be fixed.
Select employees that should be notified and be able to follow up on this corrective action.
Click on "Save" button to save all information filled in the Corrective Actions form.
After saving the modal window will be closed and you can continue with filling checklist.
In a question for which a Corrective Actions form has already been created, the "Create Corrective Action" button will be inactive and a message will be displayed that the deviation task has already been created.Created Corrective Action task can be found in a specially created deviation group for Corrective Actions tasks.
